ASFA requires courts to conduct a permanency hearing within 12 months of the child’s entry into foster care in order to determine the child’s permanency plan. ASFA also requires that the public child welfare agency make “reasonable efforts” to finalize a permanency plan for the child. A change in the child’s permanency plan and any activities, resources, and services related to that plan would be recorded within the individual child’s case record.
